".pdf" file extension. If you don't have a reader --
download a free copy from
Adobe. Several of the forms consist of multiple pages (Application - 4 pages; Physical
Exam Form - 2 Pages) After downloading a form, open it using Adobe
Acrobat Reader . Once you open the form in Adobe
Acrobat Reader, you must print it on a laser or high quality inkjet printer. Dot Matrix
forms will NOT be accepted. The forms are designed to be
printed and filled in by hand or using a typewriter. Documents or forms that have a
".doc" file extension are in Microsoft Word format. You can read and print .doc
files using most popular word processing programs. They can also be read and printed using
the free Microsoft
Word Viewer. |
